This is a sales order form from clothing manufacturer, Stanley DeSantis. Back in the early 1990's, Stanley DeSantis produced a line of MAD Magazine t-shirts and hats. There were several different styles of t-shirts, and different colors for each style also were produced. There was only one style of hat produced, and is featured a silk-screened image of Alfred E. Neuman on the front. The book, Collectibly MAD, indicates that Stanley DeSantis also produced a line of MAD Magazine sweatshirts, but I have never come across one, nor does this order form show any options for ordering a sweatshirt. This item is very cool, as it shows all of the styles of MAD t-shirts that were offered for sale.
