This is an official entry form from a sweepstakes event with Tang Breakfast Drink and MAD Magazine. This sweepstakes drawing for this event was held on July 31, 1997, and the Grand Prize was the winners likeness illustrated on the cover on a Special Issue of MAD. There was only one Special Issue published, and that was for the winner only. Measuring approx. 3 1/8" by 5 5/8".
